Once the request is received, our team will respond to confirm receipt and communicate by email with any follow up questions, verifications of the request, status of the request, etc.
- Remove Account & Data: If your request is one that you wish to remove your account and data, all data that is entered in the sb health application by you will be removed, along with all components of your account for the app. This is a permanent action and cannot be undone.
- Account Correction: Do you see something wrong with your address, or personal information? You can request updates below and we will verify that information and make the updates on your behalf.
- Account Merger: Did you have an account with another employer or group and need to merge it into your new employer/group? That can be handled by reaching out to us using the form below. We will verify all of your details and help you with this request.
